I agree that neither the NSS, the CDS nor its Board of Directors or Training Committee members are responsible for any loss of income which may result should my instructor status be suspended or revoked. *

I further agree this renewal is subject to my completing an NSS- CDS Instructor Update as well as any instructor requalification process which may be required by the Training Chairman. *
